The medical data form is waterproof, tear resistant, lasts indefinitely and is a mini personal medical record.  Data is simply printed on the form, preferably with a pencil, and this allows for easy updating by erasing old info and inserting new.  The adhesive backed black vinyl pouch is 2 1/4" x 1 1/8". There is a "DANGER Do Not Remove Helmet warning on the pouch designed so a passerby does not cause further injury.
